Next Generation Ambulatory Pharmacy

Ambulatory pharmacies have long recognized the potential for expanding their role in patient care. Nearly ten years ago, pharmacists at a Level I Trauma Center in Minneapolis began to review prescriptions for patients about to be discharged. By correcting the errors they found, the pharmacists made a significant impact, as the quality of outcomes and related costs were substantially improved: ED visits were lowered by 30%, readmissions by 42%—and costs decreased by $2,500 per patient.
